With its wide range of possibilities, it's no surprise that Auricular Therapy is a "trending" modality. I understand that the therapist is a licensed professional providing TMB services within their scope of practice as defined by the NCBTMB and regulations of professional licensure of the State of Illinois.
I acknowledge that the therapist is not a medical physician and does not diagnose illness, disease, or prescribe medication. I clearly understand that TMB is not a substitute for medical treatment or medications. I have received permission from my physician to receive TMB.
I understand no assurance or guarantee has been provided to me as to the results of my treatment. I understand the risks associated with TMB include, but are not limited to: superficial bruising, short-term muscle soreness, exacerbation of undiscovered injury, and therefore release the company and the individual therapist from all liability concerning these injuries that may occur during the TMB session. I understand there may be additional risks based on my current or previous medical conditions.
I understand that the therapist will adjust the treatment protocol based on my medical history.

If I cancel, reschedule, or fail to show up for an appointment with less than 48 hours notice, I will be charged a cancellation fee of $75/hour.
